If it is postal information you are entering all you need is HARROW for the post town/city. Middlesex is the former postal county, and you don't need to put that unless the form insists on one.

Kenton Road (with the house number) is usually the first line of the address - usually refered to as street
Harrow would be considered your town/city
Middlesex is the county (equivalent of a state in the US but on a much smaller scale in most cases)
